The office will be closing permanently as of January 30, 2024.  Dr Charles Slavik is retiring and Dr. Mike will be joining Mundelein Pediatrics, SC in Grayslake.  Dr. Mike would be very honored with any patients who would like to follow him to his new practice.  It has been a pleasure for both doctors to serve the Schaumburg and Buffalo Grove communities and build great relationships with you all.​

To request a copy of your child's records, please email your request to TOTDOC1@HOTMAIL.COM.  If you would like just a copy of immunization records, there will be no fee and we can email those to you.  For complete records, there is a standard $20 processing fee per child and a $20 total postage fee.  Please do not call Mundelein Pediatrics for your child's records from Pediatric & Adolescent Center as they are a completely separate business and will not have your child's records.
